Office Policies

 

Appointments

 

Please call us during regular office hours to make an appointment. Appointments can be made with any of our healthcare providers. When calling, please tell us the general reasons for your visit so we will know how much time to allow for your appointment. We make every attempt possible to make appointments in a timely manner.
 
If you are unable to keep your scheduled appointment please notify us as soon as possible to avoid a $25.00 fee. We will be glad to reschedule you at your convenience.

Prescription refills

 
We refill prescriptions only during office hours. Bring your medication bottles on every visit to the office so that refills can be accurately made. For your convenience, we will either give you prescriptions at the time of your visit, fax prescriptions, or e-prescribe prescriptions.
 
Patients are required to see healthcare providers on a regular basis in order to qualify for medication refills. Patients who are unable to keep scheduled
appointments may be granted a refill for a limited supply of medication. Failure to keep appointments after this may result in denial of additional refill requests.
 
Controlled substances will not be refilled over the phone or through the pharmacy.

 

Prior Authorizations

 
Insurance companies frequently deny payment for certain non-formulary
prescriptions. Our staff will make every effort to conduct a “prior-authorization” on your behalf so that you can receive your medication. This process may take a few weeks. You are invited to inquire about the status of your “prior authorization”  with us if your pharmacy refuses to dispense your medicine.

 

Phone Calls

 
All patients are encouraged to call us with any and all medical questions and concerns. Our receptionist will relay your message to the appropriate clinical staff. We will make every effort to return your call within a 24 hour period. Patients with any medical emergency should seek immediate medical attention by calling 911, going to a hospital Emergency Department, or coming to our office.

 

Billing & Insurance

 
Co-pays and any other previous outstanding balances are due at the time of service.
